About Us

Learn how we work on Improving Patient Access and Coordination through Technology

About Us

The COVID-19 pandemic highlighted the importance of collaboration among clinicians and administrators to address specific needs and challenges associated with providing care for people in congregate communities. We aim to build on this community of practice with the Virginia IMPACT initiative through engaged learning and facility-level telehealth optimization.

Mission

The Virginia IMPACT initiative aims to improve healthcare equity, accessibility, integration, and coordination through engaged learning and optimized telehealth services across congregate care communities.

Community

This project strives to bring together a community of practice of clinical, administrative, and technology professionals across the spectrum of congregate care.
